By registering my child today and signing below, I acknowledge that I am personally responsible for any damage to the facilities or equipment, or any sickness/injuries sustained by my child/children or myself while in attendance at F.A.I.T.H. events. I agree to remain in the building during class times unless prior arrangements are made and approved by the Director/Co-Director for substitute supervision of my child/children. I have read and understand the rules/regulations of the F.A.I.T.H. Handbook and will abide by them and see that my child/children abide by them as well. I also give permission for photos of my family to be used in the yearbook and on F.A.I.T.H.'s private social media.

Credit card payments can be accepted the first day of class.

Tuition Payment: the entire fee is due and payable upon registration or the first day of class, unless other arrangements have been made. Tuition is NON-REFUNDABLE.
