Board to see that everyone can play, that wishes to play, regardless of financial difficulties, please call to discuss any hardship situations – Deanna Ervin (810)625-2109. Players will be placed on teams according to skill level, age and number of registrants, not necessarily grade.

By registering my child today, and signing below, I am acknowledging that I am personally responsible for any damage to the fields or courts and any injuries sustained by my child or myself. In the event of an emergency and medical attention is necessary I give permission to the F.A.I.T.H. Director and/or Coaches to seek medical attention for my child.

Uniform & Player Fees (PLEASE READ ALL INFORMATION)

A UNIFORM WILL NOT BE ORDERED UNTIL PAID FOR. ORDERS WILL NEED TO BE PLACED BY THE TIME OF THE FIRST PRACTICE. DON'T BE THAT PLAYER WITHOUT A UNIFORM! We want our teams to feel and look “official” so we will be using team “uniforms” at all games, which will consist of two team shirts (one white & one navy blue), shorts and two pair of socks; navy blue and white. The uniforms will be purchased by the F.A.I.T.H. Sports Program; however, each player will be responsible for purchasing full shin guards and soccer cleats. Players will keep their uniforms and be responsible for laundering between games. Uniforms will be used for consecutive years of play and only the player fee will be paid each year following unless a replacement is needed.

A UNIFORM WILL NOT BE ORDERED UNTIL PAID FOR. ORDERS WILL NEED TO BE PLACED BY THE TIME OF THE FIRST PRACTICE. DON'T BE THAT PLAYER WITHOUT A UNIFORM! We want our teams to feel and look “official” so we will be using team uniforms at all matches. Polo shirts will be ordered by the F.A.I.T.H. Sports Program and paid for by the player. The player’s shirt will have the FAITH Falcons logo on them and will be used for consecutive years of play so keep this in mind when ordering sizes! We will also offer a FAITH Falcons matching hat, if desired. It is the responsibility of the player to purchase khaki shorts or pants, golf shoes (if desired) and their own clubs. Players will keep their uniform shirt and be responsible for laundering between matches.

A UNIFORM WILL NOT BE ORDERED UNTIL PAID FOR. ORDERS WILL NEED TO BE PLACED BY THE TIME OF THE FIRST PRACTICE. DON'T BE THAT PLAYER WITHOUT A UNIFORM! New players are required to purchase Jerseys and Shorts. The player’s uniform will be used for consecutive years of play so keep this in mind when ordering sizes. Uniforms Will consist of shirt and shorts. Players will keep their uniform and be responsible for laundering between games. (Jersey's are ordered "loose fit" from size chart) The player fee includes free entry for family(parents and siblings) to all FAITH Home games for the current season.
